Preble County Public Health is offering Flu Vaccines by appointment only this year. This will be starting on October 1st, 2020.  Please call 937-472-0087 ext. 226 to schedule.

You are asked to bring your insurance card and a completed questionnaire, for each person receiving the vaccine. The questionnaire is found below.
